AcroForm fields don't display.

I posted a version of this topic before, but the forum seems to have reverted. Please forgive me if the topic is still around here somewhere, because I can't find it if it is. (This is a different problem then when adding fields would make the page disappear.)

My problem is that when I create a PDF with fields, the fields don't show up in Acrobat Pro or Reader. (I haven't tried any other viewers.) I have looked at the simplest PDF I could find with a form field, and I can't find any relevant differences. I am probably just not noticing something obvious. Any page content that I add displays fine. I will paste the PDF data below. Please keep in mind that this forum somewhat distorts PDF data (most notably by removing ">" symbols), but it should still be generally readable. If you would like me to email the actual PDF to you, I would be glad to.
